A Certificate of Insurance (also known as a COI) is a document from an insurance agent/broker that serves as evidence of insurance coverage; it outlines the type of insurance, the limits of liability and the effective and expiration dates of coverage. It is important to note that a certificate of insurance is only evidence of coverage in effect at the time of issuance of the certificate; it is not an insurance policy nor does it convey any rights to the certificate holder.
When a Vendor Requires a Certificate of Insurance
Certificates of Insurance are often requested by outside parties with whom Willamette University contracts. Examples of when WU may be required to provide a certificate of insurance when: using of an off-site location to host an event; entering into a contract for services; leasing or renting real estate or equipment; and accepting a grant award.

When Willamette University Requires a Certificate of Insurance
A properly completed Certificate of Insurance must be obtained before services may be provided or others are permitted to use WU facilities.
The Certificate of Insurance must have the following: |
Willamette University, its officers, board members, employees and agents are included as additional insured. |
